Stroke Care For Your Loved One

What is a Stroke?

A stroke may be caused by a blocked artery (ischemic stroke) or the leaking or bursting of a blood vessel (hemorrhagic stroke). Some people may experience only a temporary disruption of blood flow to the brain (transient ischemic attack, or TIA) that doesn’t cause permanent damage (Source: Mayo Clinic) Others, who have a stroke may experience temporary or permanent disabilities.

There are many types of strokes including ischemic stroke, hemorrhagic and transient ischemic attack (TIA). They are all different and their treatment options vary.

Our Customized Program

We provide an individualized and customized care program to support you or your family member. >We will start with a Home Assessment and then evaluate the following:

  • Speech or language
  • Balance and vision
  • Dietary needs
  • Emotional support
  • Support for cognitive changes

In-Home Care

As needs change, we are flexible and can modify your care plan.  For example, if swallowing becomes more difficult, our in-home caregivers can prepare nutritious meals and modify food textures and thickness so it is easier to swallow and consistent with a doctor’s advice.  This will all be customized based on the Home Assessment and doctor’s recommendations and/or feedback from a team of allied health professionals.

Additional support may include:

  • Eating
  • Transfers from bed to a wheelchair, chair, or bathroom
  • Bathing and dressing
  • Medication Reminders
  • Personal Safety Habits
  • Range of Motion & Strength Exercises
  • Monitor Signs (Another Stroke)
  • Transportation (To and from Medical Appointments or Activities)
  • Housekeeping
  • Errands
  • Companionship & Emotional Support

Next Steps, RN Assessment

  • Nurse Assessment
  • Customized Plan

Ask about our complimentary training program for caregivers which allows 1-2 hours in the rehab clinic with a PT or OT to learn the in-home care plan to support our clients.
