CarePlus supports independent living communities by providing flexible care hours onsite.

Support for Independent Living

We can help residents with daily living needs including but not limited to:

Escort to Medical Appointments
Light Housekeeping
Medication Reminders

Bathing/Dressing
Meal Prep
Home Assessments

Schedule as little as 30 minutes of assistance daily, weekly or whenever it is needed– our team will be there!

Wellness Center & Programs

CarePlus can set up a permanent wellness center or a wellness pop-up on the premises. We can do onsite blood pressure checks, Lunch & Learn series, exercise programs and more. We also have expertise in virtual forums including Zoom educational programs, health activities, games and more.

Care Coordination Program

Our CarePlus coordinators can help support the coordination with out of town families, rehabilitation and other logistical needs. We also are able to support residents with hospitalizations, follow-up medical appointments, prescriptions and transitions from hospital to home.
