Clinical Care Services

CarePlus offers educational workshops and wellness programs for staff, residents, and families. Programs focus on common health concerns facing older adults and provide useful information that can be put into practice. Topics may include:

  • Lunch & Learns on current health and aging topics
  • Fall prevention
  • Blood pressure checks and monitoring
  • Healthy aging and wellness
  • Infection control
  • Medication awareness
  • Chronic disease education

CarePlus is committed to providing caregivers with hands-on training, ongoing education, and professional support. Our comprehensive training program includes orientation, annual skills labs, and quarterly roundtable discussions to strengthen skills and provide quality care.

CarePlus works closely with care managers, providers and medical practices to help ensure care plans are routinely updated and managed with clinical oversight.

CarePlus works with hospitals and discharge planners to help older adults return safely to their home or senior living community following a hospitalization or rehabilitation stay. We can coordinate services before discharge and provide nursing care, medication management, and personal care once the individual returns home.

A surgery or hospitalization can change the level of care needed. CarePlus can provide additional nursing or personal care during recovery or transitions.

Led by Dr. Heather Najjar, CarePlus offers education and training programs. Our registered nurses provide presentations for staff, residents, and families on healthy aging, chronic conditions, medication management, care transitions, and other topics that support the health and well-being of older adults.

Each program is customized to reflect the specific needs and interests of the community.
