Private Care in Your Senior Living Community

Senior living communities provide older adults with meals, socialization, amenities and opportunities that support an enhanced quality of life. But when a medical incident, fall, change in medication or temporary decline occurs, additional support may be needed.

That’s where CarePlus can help. Our experienced caregivers and clinical team can step in to provide personalized support, helping residents remain safe, comfortable and well cared for while continuing to enjoy the community they call home.

Sometimes the changes come on gradual, at other times it is immediate depending on the condition. When slow progression of balance, or cognition occurs, families are often left trying to figure out next steps including transitions for a greater level of care, this may require time and in order to keep your loved on in the same environment, additional care is needed. That’s where we come in.

When Things Change,
We Step In

CarePlus works closely with families, community staff, and healthcare providers when a resident’s needs change and additional care or attention may be needed, including:
Returning from a hospital or rehabilitation stay
Recovering from surgery or a procedure
An unexpected fall or change in mobility
Changes in medications, meals, or care instructions
A change in level of care while new arrangements are being made
One-on-one support following a stroke, hip fracture, or other health event
Additional care while waiting to transition to a higher level of care

Our team is here to support you.

Our caregivers and clinical team are there to support, not replace, the services already in place. We stay in communication with everyone involved so nothing gets missed and families feel informed. Support may include:
Help with activities of daily living
Support with mobility and fall prevention
Medication reminders and monitoring
Skilled nursing and clinical oversight
Companionship
Meal prep
Transportation to medical appointments and daily activities
Recovery support including rehab and hospital stay
Hospital admittance and support

We’ve got this, been doing it for over 30 years.
