CarePlus Home Health is a home care agency specializing in the placement of private duty caregivers (RN’s, LPN’s, CNA’s and Companions). We are seeking qualified caregivers with the highest standards of care. If you’re a great caregiver and would like to join our team, please follow these steps:
- Check the required documents below to be sure you have them all.
- Fill the initial application form below. You will receive an email within 2 business days with further information.
- When you come to your interview, you must bring your ORIGINAL, required documents. PHOTOCOPIES ARE NOT ACCEPTED.
Required Documents
- Current Driver’s License or state issued ID card
- Social Security Card
- Current Green Card or Work Permit if not U.S. Citizen (If US Citizen, bring proof)
- Current State-issued RN, LPN, GNA, or CNA license (not expired)
- School Training Certificate
- Current Police Background Check for the state(s) you’ve lived in (within 1 year)
- Current CPR Certificate (not expired)
- Current Health Screening, TB Test Results or Chest X-Ray (within 1 year)
- Three (3) Employment References with phone numbers, addresses, dates of employment, and at least 1 company reference. NO CELL PHONES for facilities or agencies. Phone numbers must be the main telephone to the location.
